Job Description
The Case Manager/Housing Navigators provide support and guidance to homeless individuals and families to bolster areas within their lives to increase stability. Key responsibilities include conducting housing eligibility assessments using the CARS Assessment, documenting data into HMIS, facilitating case planning sessions, conducting regular client meetings to track progress, maintain confidentiality and boundaries, collaboration with agencies such as Social Services, Monterey County Office of Education, Property Managers, and others, as well as documenting all Case Management and Housing Navigation activities. Other duties as assigned.
Salary $51,891 – $63,075 (40 hours per week) D.O.E, Additional 5% Bilingual Stipend
