WeCare Medical Specialty Group is seeking a compassionate and dedicated Remote Dialysis Social Worker to join our growing team. The Dialysis Social Worker will provide essential psychosocial support and case management services to patients undergoing dialysis treatment. This remote position offers a unique opportunity to make a significant impact on the lives of patients while enjoying the flexibility of working from home.
Responsibilities:
Conduct comprehensive psychosocial assessments of patients and their families to identify their needs and develop individualized care plans.
Provide counseling and support to patients and their families to address emotional, social, and financial challenges related to dialysis treatment.
Educate patients and their families about kidney disease, treatment options, and available community resources.
Collaborate with the interdisciplinary healthcare team, including physicians, nurses, and dietitians, to ensure coordinated and holistic patient care.
Advocate for patients' rights and access to resources, including financial assistance, transportation, and housing.
Maintain accurate and timely documentation of patient interactions and interventions.
Participate in quality improvement initiatives and contribute to the development of best practices.
Stay up-to-date on current trends and developments in dialysis social work.
Adhere to all HIPAA regulations and maintain patient confidentiality.
Qualifications:
Master's degree in Social Work (MSW) from a CSWE-accredited program.
Current and unrestricted license as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) in the state of residence.
Minimum of 2 years of experience in social work, preferably in a dialysis or healthcare setting.
Strong understanding of the psychosocial impact of chronic illness and disability.
Excellent communication, interpersonal, and counseling skills.
Ability to work independently and effectively in a remote setting.
Proficiency in using electronic health record (EHR) systems and other relevant software.
Must reside in the United States or have a valid US work permit.
