Job Description
PACE NURSE INTERN
Under the supervision of the RN, the PACE Nurse Intern will provide holisitic community- based care to senior participants in a PACE SEMI center, ambulatory care clinic, community and/or home setting. This is a paid summer intern position open to current nursing students in their last year of school.
Objectives of Nurse Internship
• Introduce nursing student to PACE model of care.
• Understand role of the nurse in primary care, case management, transitions of care, population health, and utilization management
• Using the nursing process enhance nursing skills in assessment, interventions, critical thinking, and care planning
• Successful completion of internship may lead to possible per diem position until completion of nursing school and consideration for full time RN position upon graduation and completion of NCLEX
Duties and Responsibilities
Under the supervision of the RN:
• Assess, establish and goals, provide interventions, and evaluate care plan and outcomes. Document in Electronic Medical Record
• Participate and collaborate with the Interdisciplinary Team to develop, implement and monitor plan of care. Primary focus is keeping seniors independent with support in their home.
• Assist with transitions of care and discharge planning.
• Perform interventions such as wound and ostomy care, catheter care, and point of care testing
• Provide education on healthy living and chronic disease management
• Provide supportive care at end of life
• Participate in process improvement initiatives
• Assist with care in the ambulatory care clinic
• Other duties as delegated
Minimum Requirements
• Currently enrolled in an accredited nursing program
• Completion of Med/Surg I
• GPA 3.0
• Current BLS
Working Conditions
• Frequent standing and bending. Potential lifting of up to approximately 40 pounds
• Some driving within designated catchment area is required. Must have reliable transportation—mileage is reimbursed
• May be exposed to infectious material or conditions, blood-borne pathogens, and/or hazardous waste. May be exposed to uncontrolled home environment
