The Social Worker will be a key participant in member interdisciplinary teams, weighing in on a host of medical, physical, social, and emotional assessments and Plans of Care. Additionally, this professional will oversee a number of core programming and therapeutic responsibilities. Duties and Responsibilities include serving as the primary liaison to community hospitals, social service agencies, mental health centers, and intensive case-management units regarding referrals for admission and discharge planning; participating in program review, quality assurance, utilization review, and staff development; coordinating admissions, re-admissions, and discharges; providing counseling to individuals and families; clinical supervision to Recovery Support Practitioners and other staff; performing administrator on-call duties including crisis response and staffing; establishing positive relationships with family members; coordinating referral processes and conducting psychosocial assessments; providing financial management assistance; utilizing safe crisis management techniques; serving as liaison to the Court for involuntary commitment hearings; communicating complaints and rights violations to senior management; and other duties as assigned. This position has no supervisory responsibilities. Physical demands include lifting, pulling, squatting, kneeling, crawling, crouching, climbing, stooping, and proficiency in CPR.
