Additional Information About the Role
- Schedule: Hours are 8-4:30 M-F, but LOV team members flex their time when they need to meet with their patients other than those hour
Main Duties:
- Role outreaches individuals who have experienced violence and have been seen in the BJH ED and referred by BJH social work team
- Role maintains caseload of 12-15 patients, documents outreach, case management interventions in Redcap
- Candidate will split their time between offices at the Delmar DiVine, BJH ED and meeting with the patient in the community
- Role connects patients to victims of violence resources and other community resources with goal of reducing repeat violence incidences for that individual
- Role works closely with the BJC Community Health Improvement Department (CHI), and is jointly supervised by WU Institute for Public Health LOV leadership (for case management supervision) and BJH leadership for BJH related needs and compliance
- Role cross covers other LOV sites as needed (SLCH, Cardinal Glennon, SLU Hospital)
Overview

Recognizing its excellence in nursing care, Barnes-Jewish Hospital was the first adult hospital in Missouri to be certified as a Magnet Hospital by the American Nurses Credentialing Center.
The CM Services reports through the division of Patient Care Services lead by the Chief Nursing Executive, Vice President of Patient Care Services. The CM Service is comprised of three departments: Case Management, Utilization Management, and Social Work. Through the Triad Model of Care Delivery, the objectives of the CM Services are for all patients to have access to healthcare through the BJH continuum and appropriate, safe discharge plans. The SW department is open 24 hours/day while the CM department is open 20 hours/day. Both departments provide coverage throughout the hospital including the Emergency Department, Outpatient Clinics, Registration sites, and Health Information Management. The Service employs registered nurses, social workers, a license practical nurse, a customer service representative, and administrative assistants.
Preferred Qualifications
Role Purpose Under direction of a physician, assists patients and their families in their residences or outpatient/inpatient settings to adapt and plan an interdisciplinary approach to treating the patient-family system.Employees working at an Illinois facility are required to have an Illinois state social work license or be able to take and pass the Illinois state social work licensing exam within 90 days of the first day of employment.
Responsibilities Provides direct clinical services to patients and families.
Plans and implements social work services on assigned clinical service or unit in collaboration with the healthcare team.
Collaborates with community human service agencies to meet the needs of patients, families, and the hospital.
